History
My basic undergraduate degree is from the Faculty of Medicine, University of Malaya, Kuala Lumpur (1985-1990 ). Nominated by Faculty of Medicine and awarded Association of Commonwealth Universities Medical Electives Bursary as an undergraduate medical student ( International Diarrhoeal Disease Centre, Dhaka, Bangladesh ). My housemanship was completed in the then University Hospital, Kuala Lumpur ( One year ). Obtained training under Datuk Emeritus Professor Dr. V. Sivanesaratnam, FRCOG for MRCOG ( UK ) in the University Hospital, Kuala Lumpur at the same time as the MOG ( Malaya ). Passed both in 1997. FRCOG ( UK ) conferred in 2010 by the Royal College of Obstetricians & Gynaecologists, United Kingdom. Subspecialty training in Minimally Invasive Surgery ( MIS ) from attachment in Fellowship Training, MIS Unit, Kandang Kerbau Women’s & Children’s Hospital, Singapore ( 3 months, 2000 ) and Aileen Dicken’s Bequest, Overseas Fund Award, RCOG under Professor Christopher Sutton/Mr. Andrew Kent, University of Surrey/Royal Surrey County Hospital NHS ( MATTU – Minimal Access Therapy Training Unit - 3 months ).

In 2003/2004 was trained in the Fellowship Training Programme ( Paid leave from UM, promoted to Associate Professor in the University of Malaya in December 2003 ), GynaeOncology Unit, Kandang Kerbau Women’s & Children’s Hospital, Singapore under A. Professor Dr. Tay Eng Hseon and A. Professor Dr. Jeffrey Low for 12 months in both complex Gynae-Oncological surgery, GynaeOncology Wards, Chemotherapy, Palliative care and Tumour Board ( GynaeOncology & CytoPathology ). Currently, Associate Professor in Obstetrics and Gynaecology in Taylor’s University, School of Medicine, Selangor as of mid-December 2013. Visiting Consultant Obstetrician and Gynaecologist/GynaeOncologist to BP Healthcare, Taman Megah, Petaling Jaya and Sri Kota Specialist Medical Centre, Klang with clinic sessions and operating/admitting rights.

A CONCEPT PAPER : TIME-LINE RECOGNITION OF CLINICALLY NON-TERMINAL NON-PALLIATIVE SUBSET OF FIGO STAGE 4 GYNAECOLOGICAL CANCERS SUBDIVIDED INTO SHORT AND LONG SURVIVORS IN THE UNIVERSITY OF MALAYA. Eugene WK Leong, A.Professor & Consultant Gynae-Oncologist, Department of Obstetrics & Gynaecology, Faculty of Medicine, University of Malaya, Kuala Lumpur, Malaysia.Objective : In cancer treatment survival and disease free interval are two milestones. Currently with treatment self selected FIGO Stage 4 cancer patients survive beyond 1 year with no disease, stable disease or delayed progression. Traditional palliation strategies do not differentiate short ( < 1 year ) and long survivors ( > 1 year ) who respond ; asymmetric management relooks at treatment for long survivors who would otherwise be given palliation only. This subset deserves clinical recognition/establishment with research, to convert short to long survivorship and for new lines of treatment. Methods : Eight long survivors with FIGO Stage 4 disease is reviewed. Results : Anoynomised data of eight patients as modeling examples who qualify as long survivors ( 7 still alive with good functional status; 1 with clinical & radiological cure ): Disease ( number; months of survival ) : Endometrial (3; 48,48,55), Leiomyosarcoma (1;24,died of disease), Endometrial Stromal Sarcoma (1;46), Ovarian cancer (1;46), Gestational Trophoblastic Neoplasia ( 1;13 ) and Poorly differentiated pelvic tumour (1;55) are reported. Asymmetrical management strategies included : IVC filter, Radiofrequency Ablation of selected lung,liver secondaries, chemotherapy,surgery,resection of bladder with isolated secondaries with ileal conduit, treatment for c-kit positive gynaecological cancer. Conclusion : Certain self selected patients upon independent review despite FIGO Stage 4 disease survive beyond expectations and palliative treatment may shortchange these patients. The bottomline is long survivors may benefit from a different approach which requires more study. Conversion of short to long survivor status is another issue.
  • 2. PRELIMINARY RESULTS: A PROSPECTIVE OBSERVATIONAL STUDY OF SURVIVAL IN PATIENTS WITH GYNAECOLOGICAL CANCER AND LUNG SECONDARIES IN THE UNIVERSITY OF MALAYA, KUALA LUMPUR, MALAYSIA.Sim BK 1 , Eugene WK Leong 2. 1 HouseOfficer, 2 Consultant GynaeOncologist & A.Professor, Faculty of Medicine, University of Malaya, K.Lumpur.Objective: Clinical observation attests that survival may not be short in patients with lung secondaries at inception and on progression,this study reports on survival of above cohort in a tertiary centre. Method:Scrutinization of data of above cohort with radiological criteria and confirmed gynaecological cancers with review of length of survival in months, coexisting secondaries and current outcome were done. Results:38 patients identified; 11 with lung secondaries at diagnosis. Primary cancers at diagnosis were Ovarian (12); Endometrial (9); Cervical and Gestational Trophoblastic Neoplasia (GTN) (5 each); Vulva/vaginal (3); Primary peritoneal (2); and others (2). Mean survival with lung secondaries at diagnosis ( months ): 13.6 (range 2-58) and in whom progressed with lung secondaries 40.3 (ranges 3-345). Respectively, mean overall cancer specific survival: cancer specific mean survival post diagnosis of lung secondaries (months) are: Cervical (96.6:3.6); GTN (24.3:16.3); Endometrial (23.9:11.3); Ovarian (15.7:4.4). Accrued coexisting secondaries : Liver(6); Bone(4); Para-aortic nodes(4); Omental secondaries(3); Spleen(2); Adrenal(1); Malignant rectovesicovaginal fistula(1). PET CT scan for anterior mediastinal mass was negative in 1 patient. There were 24 patients with confirmed lung secondaries on disease progression. 1/38 succumbed to disease. 37/38 were on treatment. Conclusion :Mean overall survival for patients with lung secondaries at diagnosis was 13.6 months and patients who progressed with lung secondaries 40.3 months. 1/38 succumbed to disease. Ongoing study of this survival data affects strategic research and may trigger new treatment philosophies in patients with FIGO Stage 4 gynaecological cancers.

TWO YEARS CANCER FREE SURVIVAL FROM FIGO STAGE 4 INVASIVE CERVICAL CANCER ( PET CT & HISTOPATHOLOGICAL CRITERIA ). Eugene WK Leong.

Associate Professor & Consultant GynaeOncologist. Objective : To report on cancer free survival from FIGO Stage 4 Cervical Cancer diagnosed in May 2014. Methodology : Madam X, a 50 year old lady presented with irregular per vaginal bleeding in May 2014 to the author. A full history, physical examination revealed a fungating cervical cancer. Cervical punch biopsy showed cervical invasive cervical cancer squamous cell carcinoma. Examination under anaesthesia, cystoscopy revealed bladder involvement. PET CT Scan evaluation confirmed the same with serosal and pelvic nodal metastases . After counselling and a Consultant Radiotherapy second opinion the patient agreed for adjuvant chemotherapy – carboplatin/paclitaxel iv ( 3 cycles by author ) followed by concurrent chemo-radiotherapy ( full dose with brachytherapy ). This was followed by another 3 cycles of adjuvant chemotherapy ( by author ). Careful follow-up ensued. Follow-up smears showed ASCUS, LGSIL, LGSIL with a hard roughened cervix with enlarged shotty inguinal lymph nodes which was very hard to tell if it was post-radiation effect or some other issue. After a repeat counselling, CT Scan, PET CT scan evaluation which were negative; patient requested for histological confirmation. This was performed by the author once ( 2016 ) – full thickness severe dysplasia on cervix. Again after conservative follow-up. Patient requested for inguinal lymph node open biopsies. Option of leave alone made known. All these, with further resection of the remnant cervix revealed no malignancy.

Results : Madam X has been continuously working at her employment despite being on treatment except for rest breaks in between.

Conclusion : It is possible to obtain negative PET CT Scan and negative histopathology for invasive cervical cancer FIGO Stage 4 after treatment by adjuvant chemotherapy and radiotherapy in Malaysia. The patient has given written consent for this report.
